We offer flexibility with our programs for special needs in terms of the hours needed to facilitate appropriate care techniques. Anything from 2, 4, 6, 8 or 24 hours can be arranged to meet your needs. Most of our programs are an out of pocket expense; however, we do accept Long Term Care insurance. Our caregivers are carefully screened through a Level II FBI process and Homeland Security check. They are also required to have their license and certifications up to date to meet state requirements. We want to put your mind at ease in knowing that our caregivers have been thoroughly screened before entering your home.
Caring for someone diagnosed with disabilities such as cancer, diabetes, high blood pressure, Alzheimer’s Disease, dementia, stroke, heart disease, asthma or other pulmonary conditions can be challenging not only for the client but the entire family. A person use to being very independent may now find they need assistance with even some of the simple functions such as their ACTIVITIES OF DAILY LIVING (ADL’S) or INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADL’S).
Our program post-discharge care can also be a powerful mechanism for preventing readmission. Home Care Services such as Personal Care Assistance/ Certified Nurse's Aide are supervised by Registered Nurse. The Registered Nurse collaborates with your Physician(s) to ensure you have the care you need. The goal is work with the staff and report to the physician(s) to ensure you get the care you need and prevent unavoidable readmission.
Cernified Nursing Asistant (CNA) Or Home Health Aide (HHA)
Services can include:
- Helping with ADL’s (activities of daily living) Bathing, Dressing and Grooming
- Light housekeeping- Laundry, cleaning, making the bed, etc.
- Meal preparation
- Medication reminders
- Emotional Support
- Assistance to Doctor Appointments
- Grocery Shopping
- Companionship and engaging in hobbies and activities
- Supervision for someone with dementia or Alzheimer’s disease
- Monitors patient condition by observing physical and mental condition, intake and output, and exercise
- Helps patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene
- Records patient information by making entries in the patient journal; notifying nursing supervisor of changing or unusual conditions
- Maintains a safe, secure, and healthy patient environment
Registered Nurse (RN) or Licensed Practicle Nurse (LPN)
Services can include:
- Wound Care
- Intravenous (IV) treatments
- Administering and supervising medications
- Pain care/control
- Instruction and emotional support
- Ventilator care
- Tracheostomy care
- Monitoring vital signs
- Ostomy/gastrostomy care
- Feeding tube care
- Catheter care
